The CRDF started using a new quarterly reporting form in August 2010. This form accommodates reports from several funding organizations. This new form works the same way as the previous FCPRAC form, except that it looks different, has one additional field, plus it offers you a prompt to save the form to your local disk when you click the Submit Report button.

Reporting Problems in Using PDF Forms
This is the first version of our new form and while we have tested successfully it in a number of different operating environments, we cannot guarantee that it will work for everyone all the time. If you have difficulty with the new form, our first suggestion is to scan the comments below to see if your issue has already been presented, or try using it from a different computer.
